I have read and understood the attached rules and regulations for the Jesus Revolution Summer Mission and agree to abide by them. I am aware that Jesus Revolution has reserved the right to send any team member and co-worker home (at their own expense) that shows blatant disregard for the stated rules and regulations.

I do hereby release Jesus Revolution, its agents, employees and volunteer assistants from any liability whatsoever arising out of any injury, damage or loss which may be sustained by me or other persons during my/their course of involvement with Jesus Revolution and participating churches for the Jesus Revolution Summer Mission.
